Admission Process
Process Outline
The admission process to Heritage Lane is simple and the majority of the effort that goes into this process is handled by our team.
- Inquiry is made to Heritage Lane
- The Admission Liaison travels to and evaluates the applicant in person and/or via Zoom, at their current place of residence and performs a general evaluation to determine basic residency eligibility
- The Admission Liaison gathers necessary paperwork and documentation and presents to the community admission team
- The Admission Team consists of:
- Director of Psychiatric Services
- Director of Clinical Services
- Business Office Manager
- The Admission Team consists of:
- An On-site tour is scheduled and hosted by Admission Liaison for the prospective resident and other individuals participating in the coordination of placement
- As part of the tour, the Business Office Manager may want to meet with financial responsible parties to ensure payment for services is secured
- Once approved, physician, equipment and medication orders are gathered for submission to ensure all proper services are in place on the day of admit
- Admission day is scheduled and Heritage Lane assists in transportation
